Authorship Statement

At the point of acceptance, all contributing authors are asked to give signed consent to publication, to confirm that they have approved the final version of their manuscript and have made all required statements and declarations. The signature box can be found on the last page of this form. If all authors are not able to sign the same copy of these statements, we are happy to accept multiple copies signed individually. It is the responsibility of the corresponding author to ensure that all sections of this document and the final accepted version of the manuscript have been read and approved by each of the contributing authors.
